- Name of Employer;
- Job Title;
- What day did you start working;
- What was the last day of your employment;
- Monthly income after tax;
- Hours worked per week;
- Job Title;
- If you have been dismissed, the circumstances which led to your dismissal;
- Details of any money that you believe that you are owed;
- If you have been a victim of discrimination, details of this.
