  • Name of Employer;
  • Job Title;
  • What day did you start working;
  • What was the last day of your employment;
  • Monthly income after tax;
  • Hours worked per week;
  • Job Title;
  • If you have been dismissed, the circumstances which led to your dismissal;
  • Details of any money that you believe that you are owed;
  • If you have been a victim of discrimination, details of this.
